Notice of Privacy Practices

The confidentiality of your health information is important to Jewish Family Services, herein referred to as JFS. JFS will maintain the confidentiality of your health information and will not disclose your information to others unless you authorize us to do so, or unless the law authorizes or requires us to do so.

A federal law, the Health Insurance Portability and Accountability Act (HIPAA) requires that JFS take additional steps to keep you informed about how we may use information that is gathered in order to provide counseling services to you. As part of this process, JFS in required to provide you with the attached Notice of Privacy Practices and to request that you sign the attached written acknowledgement that you received a copy of the Notice. The Notice describes how JFS may use and disclose your protected health information to carry out treatment, payment, or mental health care operations and for other purposes that are permitted or required by law. This Notice also describes your rights regarding health information that JFS maintains about you and a brief description of how you may exercise these rights.
